مقایسه اثر درمانی ریسپریدون و آرپیپرازول در ترکیب با والپروات سدیم در بیماران مانیای حاد یا مختلط
Comparison of the Efficacy of Risperidone and Aripiprazole in Combination with Sodium Valproate in Patients with Acute Manic or Mixed Episodes

<p>مقدمه: با افزایش تعداد عوامل درمانی مؤثر، مانیا همچنان یک مشکل چالش برانگیز برای سیستمهای بهداشتی درمانی باقی مانده است. این مطالعه مقایسه اثر درمانی ریسپریدون و آریپپرازول در ترکیب با والپروات سدیم در بیماران دو قطبی مانیای حاد یا با دوره مختلط بستری شده در بیمارستان روانپزشکی رازی تهران میباشد.</p>
<p>روش کار: این مطالعه به روش کارآزمایی بالینی تصادفی دو سو کور در دو گروه 20 نفره (18-65 سال) بیماران مبتلا به اختلال دوقطبی نوع یک در دورهٔ مانیا یا مختلط، انجام شد. بیماران به طور تصادفی، در دو گروه دریافت کننده آریپیپرازول و ریسپریدون در ترکیب با والپروات سدیم قرار گرفتند و پاسخ درمانی با مقیاس یانگ و میزان افزایش وزن در هفته 3 و 6 ارزیابی شد دادهها با استفاده از آزمونهای کای- دو، تی زوجی و تحلیل کوواریانس و اندازه گیری مکرر مورد تجزیه و تحلیل قرار گرفتند.</p>
<p>یافتهها: شرکت کنندگان از نظر مشخصات دموگرافیک تفاوت معناداری نداشتند ارزیابی پاسخ درمانی بعد از هفتههای 3 و 6 (کاهش 50 درصدی در مقیاس یانگ) در هر دو گروه اختلاف معنی داری بین دو ترکیب درمانی را نشان نداد ترکیب والپرات سدیم و ریسپریدون افزایش وزن بیشتری را در پایان هفته 6 نسبت به ترکیب والپرات و آرپیپرازول نشان داد (001/0 > P).</p>
<p>نتیجه گیری: بر اساس نتایج این مطالعه ترکیبهای دارویی ذکر شده در اختلال دو قطبی نوع یک با دورهٔ مانیا یا مختلط مؤثر و دارای اثر درمانی مشابهی بود و به طوری که هر دو درمان مؤثر و قابل استفاده است. لیکن با توجه به افزایش وزن کمتر ترکیب والپروات و آریپیپرازول در بیماران مستعد افزایش وزن، به این ترکیب به عنوان درمانی مناسب، کم ضررتر و موثرتر توصیه میشود.</p>

<p><strong>Introduction:</strong> By increasing the number of effective treatments, mania is still remains a challenging problem for health systems. The aim of this study was to compare the efficacy of risperidone and aripiprazole in combination with sodium valproate in bipolar patients with acute manic or mixed episodes who were hospitalized in Razi Psychiatric Hospital in Tehran.</p>
<p><strong>Methods:</strong> This study was conducted as a double-blind randomized clinical trial in two groups of bipolar disorder patients with manic or mixed episodes (age rage, 18-65 years). The patients were randomly categorized into two groups who received valproate with aripiprazole or risperidone. The clinical response was assessed by the Young mania rating scale and weight gain within 3 and 6 weeks. Data were analyzed using the Chi-square test, paired t-test and analysis of covariance and repeated measures.</p>
<p><strong>Results:</strong> The study participants had no significant difference in demographic characteristics. Evaluation of the treatment response after 3 and 6 weeks (50% reduction in the Young's scale) in both groups showed no significant difference between the two therapeutic combinations. The combination of sodium valproate and risperidone showed higher weight gain in comparison with the combination of valproate and aripiprazole at the end of the week 6 (P < 0.001).</p>
<p><strong>Conclusions:</strong> The mentioned therapeutic combinations in the bipolar I disorder with a manic or mixed episode have a similar therapeutic effect. Also, there was no significant difference in their efficacy and both treatments can be used. However, due to the less weight gain, the combination of valproate and aripiprazole is recommended as a safer and more effective therapy in patients who were prone to weight gain.</p>
