Introduction: Suicide is a psychiatric emergency and several factors play a role in its occurrence. Identifying these risk factors and taking measures to reduce them seems essential. Therefore, the present study aimed to model the relationships between childhood traumatic experiences, emotional dysregulation, and suicidal thoughts, considering the mediating role of mindfulness.
Methods: The present correlational study was based on structural equations. The statistical population of the study included all patients referred to clinics in Tehran in 2024, of whom 220 were selected through convenience sampling and responded to the Childhood Trauma Questionnaire (CTQ), Beck Suicidality Inventory (BSIS), Difficulty in Emotion Regulation Scale (DERS), and Mindful Awareness and Attention Scale (MAAS). Pearson correlation, structural equation modeling, and SPSS-26 and AMOS-24 software were used to analyze the data.
Results: The results showed that the direct effect of childhood trauma on suicidal thoughts (β = 0.66, T = 6.25), the direct effect of emotional dysregulation on suicidal thoughts (β = 0.76, T = 7.31), and the direct effect of mindfulness on suicidal thoughts (β = -0.58, T = -21) were significant. Also, the indirect effect of childhood trauma on suicidal thoughts through the mediating role of mindfulness was significant (P < 0.05, b = -0.47), and the indirect effect of emotional dysregulation on suicidal thoughts through the mediating role of mindfulness was significant (P < 0.05, b = -0.53). The results also showed that the research variables explained a total of 78% of the variance in suicidal thoughts.
Conclusions: Based on the results of the present study, it is suggested that special attention be paid to emotion regulation-based therapies, mindfulness-based cognitive therapy, and acceptance and commitment therapy with the aim of reducing suicidal thoughts in people who have experienced childhood trauma.

Keywords: Suicidal thoughts, emotional dysregulation, childhood trauma, mindfulness.
