Challenges and Solutions for Providing Spiritual Care in Nursing in IRAN: A Scoping Review

Introduction: Providing spiritual care by nurses to patients is essential, and on the other hand, this care is not fully implemented, so conducting a systematic review of the challenges and obstacles to implementing spiritual care in the Iranian nursing system seems necessary, so this scoping review was conducted with the aim of determining the challenges in providing spiritual care in Iranian nursing and providing solutions to improve it.
Methods: In this scoping review based on the Arksey & O'malley approach, the keywords "spiritual care", "spirituality", "nursing", "nurses" and "Iran" and their English equivalents were searched in reputable Persian and English databases including PubMed, Web of Science, Scopus, Magiran, Iranmedex, SID between 2014 and 2025.
Results: Among the 1237 articles from the initial search, finally, after screening the articles, 13 articles were reviewed, including nine quantitative articles and four qualitative articles. The findings show that Iranian nurses are aware of the importance of spiritual care, but they face several challenges in providing it. These challenges mainly include lack of knowledge and training, busy work schedules, lack of sufficient motivation, diversity of patients' spiritual needs, and lack of sufficient support from the system. Also, the reviewed studies have reported appropriate solutions, including structured and targeted education, respect for religious beliefs and convictions, the supportive role of nurses and families, formation of a care team, and the necessity of spiritual care as part of comprehensive care.
Conclusion: The results indicate that despite the understanding of the importance of spiritual care by Iranian nurses, structural, educational, and individual challenges have created significant challenges in the effective provision of this care. Addressing these challenges requires comprehensive educational planning, empowerment of nurses, and creation of a supportive environment in the health system.
