The relationship between somatic symptoms and suicidal thoughts in adolescents: The mediating role of depressive symptoms and rumination

Introduction: Somatic complaints and suicidal ideation are common in adolescents. However, studies examining the underlying mechanisms between somatic symptoms and suicidal behaviors in adolescents are limited. The aim of the present study was to investigate the relationship between somatic symptoms and suicidal ideation in adolescents through the mediation of depressive symptoms and rumination.
Methods: The present study was a descriptive-correlational study of structural equations type. The statistical population of the study consisted of all adolescents in Kermanshah city in 2025. From the statistical population, 410 people were selected by convenience sampling method. The research tools were: Patient Health Questionnaire, beck scale for suicidal ideation, Beck Depression Inventory-II, and Ruminative Response Scale. Pearson correlation, structural equation modeling, and SPSS version 23 and AMOS version 23 software were used to analyze the data.
Results: The findings indicated that the desired model fit and the somatic symptoms variable explained 71% of the variance in suicidal ideation with the mediating role of depressive symptoms and rumination. The results indicated that the indirect effect of somatic symptoms on suicidal ideation through the mediating role of depressive symptoms (p<0.05, b=0.63) and rumination (p<0.05, b=0.71) was significant.
Conclusions: Somatic symptoms directly and through the mediation of depressive and ruminative symptoms predict suicidal ideation in adolescents. This finding emphasizes the need to assess and manage somatic complaints and add clinical interventions that aim to address depressive and ruminative symptoms to prevent suicide among adolescents.
