Comparing the Effectiveness of Emotional Schema Therapy and Dialectical Behavior Therapy on Experiential Avoidance in Maltreated Adolescents

Introduction: Adolescents are in a period of psychological growth and development, and unsafe experiences of maltreatment can have profound effects on their mental health. In this regard, the present study aims to compare the effectiveness of emotional schema therapy (EST) and dialectical behavior therapy (DBT) on experiential avoidance (EA) in maltreated adolescents.
Methods: The present study was a quasi-experimental study with a pre-test-post-test design and a control group with a two-month follow-up period. The study population consisted of male high school students in Hamadan city during the 2024 academic year. 45 people were selected from three schools using non-random sampling and randomly assigned to three groups: experiment 1 (15 people), experiment 2 (15 people), and control (15 people). Acceptance and Action Questionnaire – version 2 (AAQ-2) was used to collect data. EST based on the Leahy et al. (2002) protocol and DBT based on the Linehan (1987) model protocol were each implemented in groups for eight weeks of 90 minutes. Data were analyzed using repeated measures analysis of variance using SPSS-24 statistical software.
Results: The results showed that there was a significant difference between the groups in the variable of EA (P=0.01). The results of the Bonferroni test for between-group comparison showed that there was a significant difference between the EST and control groups (P=0.01) and DBT and control (P=0.01); in fact, both treatments had a significant effect on reducing EA compared to the control group until the follow-up stage; however, there was no significant difference between the two treatment groups of EST and DBT (P=0.67).
Conclusions: The findings indicated that emotional schema therapy and dialectical behavior therapy interventions had a similar role in reducing experiential avoidance in maltreated adolescents; accordingly, implementing these interventions is recommended to reduce EA and improve the psychological health of maltreated adolescents.
