Introduction: Female pre-hospital emergency personnel often experience a heavy workload, strict organizational regulations, and insufficient time to cope with job-related challenges. Sleep deprivation and poor sleep quality are very common in this group. Therefore, the present study was conducted to investigate the relationship between job stress and chronic fatigue with sleep quality among pre-hospital emergency personnel in Tehran, with the mediating role of rumination.
Methods: This study was descriptive correlational research. The statistical population included all female pre-hospital emergency personnel in Tehran during the year 1402–1403 (N = 200), from whom 160 individuals were selected through convenience sampling. The participants completed the Pittsburgh Sleep Quality Index (1989), the Harris Job Stress Questionnaire (1995), the Chalder et al. Chronic Fatigue Scale (1993), and the Nolen-Hoeksema Rumination Questionnaire (1991). The collected data were analyzed using SPSS and AMOS software version 24, employing Pearson correlation coefficient and path analysis.
Results: The results showed that job stress (r = -0.51, P < 0.01), chronic fatigue (r = -0.48, P < 0.01), and rumination (r = -0.43, P < 0.01) had significant negative relationships with sleep quality among female pre-hospital emergency personnel. Additionally, rumination played a significant mediating role in the relationship between job stress (β = -0.60, P < 0.01) and chronic fatigue (β = -0.55, P < 0.01) with sleep quality. The model also demonstrated a good fit.
Conclusions: The results of the present study indicate that job stress and chronic fatigue can play a significant role in predicting sleep quality both directly and indirectly through rumination. Therefore, the findings highlight the importance of reducing rumination in preventive and therapeutic interventions related to sleep quality among pre-hospital emergency personnel.
