The Relationship between Cultural Competence and Spiritual Health among Nurses Providing Home HealthCare Services

Introduction: Providing healthcare and nursing services in home settings in Tehran involves considerable cultural and spiritual challenges. In this context, nurses’ cultural competence and spiritual health play a critical role in improving the quality of care and promoting both patient and nurse well-being. The present study aimed to investigate the relationship between cultural competence and spiritual health among nurses delivering home healthcare services in Tehran.
Methods: This descriptive-correlational study was conducted during the spring of 2025 on a sample of 283 nurses offering home healthcare services in Tehran. Participants were selected via non-random convenience sampling from among nurses actively working in the home care field, with a minimum of six months of documented clinical experience. Data collection tools included a demographic questionnaire, the Cultural Competence Scale developed by Naghizadeh et al. (2020) with a Cronbach’s alpha of 0.96, and the Spiritual Health Scale by Paloutzian and Ellison (1982), with a Cronbach’s alpha of 0.82. Data were analyzed using SPSS version 26, employing Pearson correlation and linear regression analyses.
Results: The majority of participants were male (62.2%) and held a bachelor’s degree in nursing (80.9%). The mean score for cultural competence was high (M = 203.11, SD = 24.18), and the mean score for spiritual health was moderate to high (M = 88.96, SD = 19.18). A significant positive correlation was observed between cultural competence and spiritual health (r = 0.344, p < 0.01). Furthermore, linear regression analysis revealed that cultural competence was a significant predictor of spiritual health (R = 0.344, p < 0.001, R² = 0.118, Adj. R² = 0.109).
Conclusion: The results of this study indicate a significant and positive association between cultural competence and spiritual health among nurses in the study population. In other words, nurses with higher levels of cultural competence tended to report greater spiritual health. Enhancing cultural competence may thus serve as an effective strategy for promoting the spiritual health of nurses and, in turn, improving the quality of home healthcare services.
