Introduction: Multiple Sclerosis (MS) is one of the chronic neurological disorders in which emotion regulation plays a significant role. Since meta-emotional dimensions (emotional reactions to primary emotions) are crucial in this process, the present study aimed to compare the effectiveness of Emotion Efficacy Therapy (EET) and Compassion-Focused Therapy (CFT) on meta-emotions in women with MS. Methods: This study employed a semi-experimental design with a pre-test, post-test, and three-month follow-up, along with a control group. The statistical population included all women diagnosed with Multiple Sclerosis who referred to MS specialized centers in Isfahan during the second half of 2023. Considering potential participant attrition, a sample of 54 women with MS was selected using convenience sampling. Participants were then randomly assigned to three groups: Emotion Efficacy Therapy (EET), Compassion-Focused Therapy (CFT), and a control group (18 participants per group). Both interventions were conducted in eight 90-minute sessions held weekly. The research instrumentwas the Metamood Questionnaire by Mitmansgruber (2009). Data analysis was performed using repeated measures ANOVA with SPSS version 26. Results: The findings revealed significant differences in the scores of positive and negative meta-emotional dimensions—including facets of meta-compassion, meta-interest, meta-anger, meta-shame, and meta-inhibition—between the experimental groups and the control group in both post-test and follow-up phases (P<0.01). In other words, both EET and CFT were effective in improving meta-emotions, and these effects were maintained during the follow-up phase. However, adjusted mean comparisons showed no significant difference between the two therapy groups in any of these components (P>0.05). Additionally, in the component of "meta-cognitive control," despite observable changes in the mean scores of both therapy groups in post-test and follow-up, these changes were not statistically significant (P>0.05). Conclusion: Based on the obtained results, it can be concluded that both Emotion Efficacy Therapy and Compassion-Focused Therapy are effective in improving meta-emotional dimensions, fostering more balanced emotional responses to internal experiences in patients. The persistence of these changes in the follow-up phase further indicates the long-term efficacy of these interventions. However, regarding the component of meta-cognitive control, neither therapy had a significant effect, which may point to the cognitive complexity of this dimension and the need for more cognition-focused interventions
