The Relationship Between Childhood Trauma and Substance Use Tendency and Suicidal Thoughts in Young Adults: The Mediating Role of Alexithymia

Introduction: today, the increasing spread of drug use and suicidal thoughts has been raised as a serious threat to the health of society. the aim of this study is to determine the mediating role of alexithymia in the relationship between childhood trauma with substance use tendency and suicidal thoughts in young adults.
Methods: The research method was descriptive-correlational. the statistical population in this study included all young adults referring to clinics and psychological service centers in tehran in 2025, from which 250 individuals were selected using convenience sampling. participants completed the questionnaire childhood trauma, addiction potential scale, beck suicidal ideation scale, and toronto alexithymia scale. data analysis was performed using structural equation modeling with spss-24 and amos-26 software.
Results: The results showed that the research model has a good fit. The indirect effect of childhood trauma on substance use tendency through the mediating role of alexithymia is significant (P<0.05, b = 0.65) and the indirect effect of the childhood trauma on suicidal thoughts through the mediating role of alexithymia is significant (P<0.05, b = 0.63). The results also showed that the research variables explained a total of 46% of the variance in substance use tendency and 49% of the variance in suicidal thoughts.
Conclusions: considering the impact of childhood trauma and alexithymia on suicidal thoughts and substance use tendency in young adults, it is suggested that educational workshops be considered to increase awareness about emotion regulation and management and to teach scientific strategies to improve emotion management for individuals exposed to traumatic events.
