Introduction: Borderline personality disorder is one of the most complex and challenging mental disorders, characterized by severe mood swings, difficulties in interpersonal relationships, and unstable patterns of self-perception. The present study aimed to compare the effectiveness of dialectical behavior therapy and stress-based mindfulness on high-risk sexual behaviors in individuals with borderline personality disorder. Methods: The research method was a quasi-experimental design with a pre-test-post-test design with a control group and a three-month follow-up period. The statistical population consisted of 92 women with borderline personality disorder who referred to Bahar and Avay No counseling and psychotherapy centers in Rasht in the first six months of 2024 in Rasht. 45 eligible individuals were selected based on inclusion and exclusion criteria and through purposive sampling and were randomly assigned to two experimental groups and a control group. The data collection tools were the Sexual Risk Behavior Questionnaire (Zarei et al., 2010) and the Borderline Personality Disorder Questionnaire (Leishnering, 1999) and a structured clinical interview. Both experimental and control groups were pre-tested using the research tools. Then, stress-based mindfulness intervention (Kabat-Zinn, 2009) for 8 sessions and dialectical behavior therapy (Linehan, 1993) for 10 sessions were implemented for their respective experimental groups, but the control group was placed on a waiting list. At the end of the interventions, both experimental and control groups were post-tested using the research instrument. Three months later, a follow-up test was administered to both experimental and control groups. The data were analyzed using repeated measures analysis of variance and Bonferroni post-hoc test. Results: The findings showed that both treatments (dialectical behavior therapy and stress-based mindfulness) were effective in reducing risky sexual behaviors, and this effect persisted into the follow-up period (P < 0.05), however, no significant difference was observed between the two treatment approaches. In general, it seems that both treatments are useful and applicable for individuals with borderline personality disorder. Conclusion: Dialectical behavior therapy and stress-based mindfulness have been shown to be significantly effective in improving risky sexual behaviors in individuals with borderline personality disorder. Therefore, specialists, researchers, and therapists in this field can use these interventions as effective options in these individuals.
