Introduction: Cardiovascular diseases affect not only physical functioning but also psychological and behavioral well-being. Concerns related to cardiovascular health may contribute to the development and persistence of psychosomatic symptoms. Despite the high prevalence of psychosomatic symptoms, no comparative study has been conducted between these two therapeutic approaches in this population.This study aimed to compare the effectiveness of mindfulness-based cognitive therapy (MBCT) and cognitive behavioral therapy (CBT) on psychosomatic symptoms among patients with cardiovascular diseases. Method:This quasi-experimental study used a pretest–posttest design with a control group and a three-month follow-up. The statistical population of the study consisted of women diagnosed with cardiovascular diseases who attended the Health and Treatment Center of District 5, Tehran Municipality, in 2025. A total of 45 participants were selected using purposive sampling and randomly assigned to three groups (n = 15 each). The MBCT intervention was conducted in eight sessions, whereas CBT was delivered in ten sessions. The control group received no psychological interventionData were collected using the Health-Related Quality of Life Questionnaire and the Psychosomatic Symptoms Questionnaire developed by Lacourt et al. (2013). Data analysis was performed using descriptive statistics and mixed repeated-measures ANOVA in SPSS version 24. Results:Significant differences were observed between the experimental and control groups (F = 52.791, η² = 0.715, P ≤ 0.01). Both interventions reduced psychosomatic symptoms in cardiovascular patients, and the effects remained stable at follow-up (P ≤ 0.01). Furthermore, mindfulness-based cognitive therapy showed significantly greater effectiveness than cognitive behavioral therapy (P ≤ 0.01). Conclusion:Both mindfulness-based cognitive therapy and cognitive behavioral therapy appear to be effective interventions for reducing psychosomatic symptoms in patients with cardiovascular diseases. Given the superior outcomes associated with MBCT, its application in clinical and psychological interventions for cardiovascular patients may be considered.
